Wills, full administration and releases

Forms are organized for decedent’s will, full administration, release from administration, short-form release, summary release, certificate of transfer, ancillary administration, claims, insolvency, reopening estates and safe deposit box matters.

Adult, minor and emergency matters

The forms list includes incompetent adult, minor, emergency guardianship, interstate transfer, sale of real property, complaint process, conservatorship, civil commitment and protective services items.

Adoption, name, birth and records

The forms page includes adoption, name change/legal name conformity, release of medical or financial records, birth correction, delayed registration, disinterment, personal injury claims, paternity, subpoenas, attorney matters, indigency and pro se packets.

$1 per page

The cost PDF lists certified copies of court documents at $1 per page. Ask whether your destination needs a certified, authenticated, exemplified or regular copy.

$0.05 per page

Regular copies may be enough for personal reference, but they may not satisfy banks, title companies, agencies, insurers or court-related requirements.

Good-cause waiver note

The cost PDF says the court may waive the deposit for costs upon good cause shown, and the required form can be obtained from the Probate Clerks or Help Center.

Legal information and selected forms

The Help Center can provide legal information and assistance with the forms you select. It can also provide information sheets on common probate matters.

No legal advice or counsel

The Help Center page says it cannot provide legal advice, legal counsel, or help if you already have an attorney for the matter to be discussed.

Generally Thursday and every other Monday

Subject to court opening and the Help Center Director’s schedule, it is generally open every Thursday and every other Monday, 9:00 a.m. to 4:00 p.m.

Use official SC and GC forms

Local rules say Supreme Court of Ohio forms and Geauga County prescribed forms must be used when applicable. The court may reject other forms if a prescribed form is required.

Fee filings by 4:00 p.m.

Normal court hours are 8:00 a.m. to 4:30 p.m., but new cases and filings requiring payment of a fee must be submitted no later than 4:00 p.m. for same-day processing.

Missing information can stop filing

The rules say filings must be complete and accurate and the court will not modify proposed filings to correct inaccurate or missing required information.

Do not expose protected identifiers

Local rules caution against filing protected personal identifiers such as Social Security numbers, account numbers, PINs, passwords and digital access codes unless required by law.

Inspection is controlled by the clerk/court

Files and records must remain in the clerk’s possession and control. Some inspection and copying requests require the court’s case-document request form and written permission.

Extra steps may apply

Guardianship rules include background-check, hearing, expense, affidavit, education and court-order requirements depending on the matter. Use the court’s current forms and instructions.

Probating original, lost or destroyed will

Use the Decedent’s Will category when the issue is admission, delivery, deposit or probate of a will. Ask the court before altering or copying an original will.

Full administration

Use this when appointment of fiduciary, inventory, account, surviving spouse rights, sale, transfer or distribution issues require an administered estate.

Release or summary release

Use release options only if the estate qualifies. The cost PDF lists different deposits for Release, Summary Release and Short Form Release.

Certificate or transfer forms

For real estate, check Certificate of Transfer, transfer of real property and land sale categories. Local rules include real-property consent and court-approval details in certain matters.

Presentation of claims

The forms page includes Claims Against Estate, and the cost PDF lists Presentation of Claim at $10. Use exact case and estate information.

Reopen estate or newly discovered assets

If assets are discovered after closure, the local rules include procedures for reporting and reopening. Use official forms before filing.

Hearing and notice details matter

Local rules say that, unless the court orders otherwise, both the applicant for guardian of a minor and the proposed ward must attend the hearing. Notice rules may apply when a minor is in the care of another person.

Background and affidavit requirements

Local rules include background-check, Guardian’s Affidavit, personal identification and jurisdiction-affidavit requirements depending on the case. Use official forms and check current instructions.

No spending without court authority

Local rules say a guardian shall not expend the ward’s property, including support, maintenance or education expenditures, until the court authorizes the expenditure by court order.

Guardian complaints and comments

The local rules describe how complaints and comments regarding a guardian are filed and handled by the court’s Guardianship Program Director or designated staff member.

Using Clerk of Courts for probate first

Geauga County Probate Court records and probate forms should start with the Probate/Juvenile Court Probate Division, not a general Clerk of Courts page, unless the court directs you there for another case type.

Treating docket search as a certified record

A docket page may help identify a case, but banks, agencies, attorneys and title companies may require certified, authenticated or specific case documents.

Using old or non-Geauga forms

Local rules say prescribed forms must be used when required, and incomplete or nonconforming filings may be declined or stricken after notice.

Missing the 4:00 p.m. fee-filing cutoff

The court may not process fee filings submitted after 4:00 p.m. until the following Court Day, even though normal court hours run until 4:30 p.m.
